The Government will award a FAR Part 12 contractual vehicle resulting from this solicitation to the responsible offeror whose offer conforming to the solicitation will be most advantageous to the Government, price, and other factors considered. The purpose of the contract is to obtain interpretation of electronic medical images. Contractor shall furnish all necessary personnel, supplies, materials, network service and equipment to perform under the terms of this contract. All services supplied by contractor under the terms of the contract shall be performed in compliance with all requirements and regulations of Food and Drug Administration (FDA), Mammography Quality Standards Act (MQSA), the Joint Commission (TJC), Centers for Medicare & Medicaid Services (CMS) and Health Insurance Portability and Accountability Act of 1996 and Privacy Act (HIPAA). This RFQ is set aside 100% for Service-Disabled Veteran-owned Small Businesses under NAICS 621512 - Diagnostic Imaging Centers. The service is to be provided for two locations identified below: Phoenix Indian Medical Center Parker Indian Health Center 4212 N. 16th St 12033 W.
