T... The service is to move, set up and install a FEMA Trailer on-site at the Kyle Health Center. The trailer will require water, sewer, electricity and data connections, tie down, skirting and a shower. The acquisition is required to provide a base operation center for the recently acquired Ambulance service with space for on-call personnel to rest and prepare for emergency calls. This acquisition is an emergency as there is currently no base operation center at Kely Health Center nor is there space available to create one within the facility. The project is an unusual and compelling urgency as the need to provide ambulance services to this remote location on the Pine Ridge Indian Reservation is necessary to transport ambulatory patients to the health center for evaluation and possible air transport to a regional trauma center. This base operation center is needed immediately therefore there is no time to seek full and open competition. They Kyle Health Center (KHC) has received a 12 ft x 60 ft FEMA trailer which is currently on site at Kyle, South Dakota. The KHC will remain open to the public 10 hours a day, 5 days a week. As a result, the contractor will be required to perform work in a manner which will minimize disruption to patient care services. Following award of the contract, a preconstruction conference will be held at the KHC. The contractor shall provide all plant, labor, materials, transportation and supplies required to complete all work and aspects of this project in its entirety. All construction shall be in compliance with 2012 NFPA 101, 2018 FGI for Outpatient Facilities and all applicable local, state and federal codes. FEMA DELIVERY AND SETUP: Delivery/Construction: The 12 ft x 60 ft FEMA trailer may need to be moved from the location it was delivered to at the KHC. The FEMA trailer shall be leveled with a +/- 0.1 tenth of plum using Polyvulc Peir Pad, structural concrete blocks and wooden shims for final grade. The existing KHC area will need to be scheduled for any minor demolition to receive the FEMA trailer on the foundation or access to existing utilities. Once the FEMA Trailer is level and sitting on the foundation the tongue of the trailer shall be cut off and disposed of per the federal government’s recommendations. Anchors for the FEMA trailer shall be installed once the trailer has been leveled and before the skirting is installed. Plumbing: There shall be a new water service connected for the FEMA trailer, which will include a service connection, 1-1/2-inch PVC CL 200 pipe, 1-inch HDPE pit, meterpit, water fittings, freezeless riser, electrical heat tape, connection to trailer stub out. There shall be a new shower installed in the bathroom and connected to the existing plumbing with ¾-inch PEX piping, copper fittings, sewer DWV pipe, etc. Electrical and Data: There shall be new electrical and data service supplied to the FEMA trailer, including an electrical meter and drop (If needed). The data cable installation will be CAT 6 cable in 1-1/2-inch PVC conduit from the Kyle Four Directions Clinic. Final connections to the existing IT system will be by others. HVAC: The FEMA Trailer has a self-contained HVAC system already installed. Sewer System: There shall be a new sewer service system for the FEMA trailer, which includes a sewer connection with a sewer wye to the existing sewer system, 4-inch DWV pipe, 2- Cleanouts, and service connection to the trailers sewer stub out. Skirting: There shall be insulated skirting constructed to the FEMA trailer with two access panels for maintenance. The FEMA trailer shall be level prior to the skirting installation. Steps and Ramps: Existing steps and ADA ramp shall be supplied by the federal government and will be installed by the contractor. HVAC and BAS: Not included in this contract. The contractor shall restore all areas disturbed by construction to a condition at least equal to that existing prior to construction, including touch-up painting where walls are scratched or damaged. Contractor shall minimize interference to the function of the Service Unit during construction. The contractor shall dispose of all excess construction materials, equipment, tools, waste, and rubbish. The project shall contain provisions for all permits, insurance and fees, federal and tribal, required of a general contractor working on the Pine Ridge Indian Reservation. B. SUBMITTALS Following contract award, the contractor shall provide the Service Unit submittals on proposed methods and materials to be utilized during the project. The contractor shall provide an electronic and paper set of submittals and samples to the Project Engineer for approval at least two weeks before a determination is required. Submittals shall include: 1-1/2-inch CL 200 PVC pipe 3/4-inch HDPE CL 200 pipe Water fittings Water meterpit Water freezeless riser Electrical heat tape Sewer Wye 4-inch DWV PVC pipe Sewer Cleanout Sewer fittings Skirting Concrete blocks Shower Anchors Base Aggregate Submittals shall conform to the applicable sections of the UBC, NFC, AIA and all Federal, State, and local ordinances. C. NOTICE TO PROCEED The final submittals shall be provided to the Facility Manager within 10 days of the notice of award. The Project Officer shall return comments within 7 days. The total project completion time will be 60 days. D. WARRANTY: Unless otherwise specified, the contractor shall guarantee all materials and work performed for a period of one year from the date of substantial completion of the project. E. SCHEDULE The work will be performed at times that will provide the least amount of disruption to the services and normal activities at the KHC. The contractor shall submit an estimated construction schedule to the Facility Manager with the product submittals. All work schedules shall be coordinated with the Facility Manager to assure a minimum of disruption. Arrangements shall be made with a minimum of 5 days notice of the intended work schedule to allow for minor adjustments in departmental routines if required. F. PROJECT REQUIREMENTS The attached drawing shows the area of placement and connections to all existing utilities, etc. This data is provided for estimation of project scope only. The contractor will be required to verify all field measurements and utilize these figures for estimation and bidding purposes. The contractor shall take adequate precautions to protect staff and patients from excessive noise, dust, vibration, or any other by-product of the project work. Following the completion of installations, all areas shall be restored to their existing condition. G. TRANSITIONS All areas of transition between existing services to the FEMA trailer will be coordinated with the local Kyle and Pine Ridge Facilities departments. Transitions shall provide for a minimal disturbance between two buildings and be as smooth as possible. Further detail is provided in the technical product specification section. H. ON SITE STORAGE Not on this contract I. TOILET FACILITIES The contractor and associated staff may use the public restroom facilities as needed with approval from the KHC. Caution and reasonable care shall be exercised to assure adequate access by patients, clinic staff, and the public to rest room areas. J. SAFETY AND SECURITY The contractor shall be responsible for the safe progress of the work to assure that patients, clinic staff, visitors, and children are not allowed access to unsafe areas or conditions during the progress of the work. The contractor and staff shall afford all staff, patients and visitors professional courtesy during the course of the project. The clinic and the Indian Health Service staff shall not be responsible for the security of the contractor’s tools, equipment, or material during the course of the project. J.
