Only quotes submitted by Small Business will be accepted by the Government. Any quote that is submitted by a contractor that is not a Small Business will not be considered for an award. Phoenix Area Indian Health Service (PAIHS) is soliciting a request for quotes to provide a Physicist in accordance with the attached Scope of Work for Service Units located under the Phoenix Area Office (PAO) located in Phoenix, Arizona. The Government anticipates awarding a firm fixed-price, FOB destination and all items must provide a quote to be considered for award. The Government anticipates to solicit, negotiate, and award a Single-Award, firm fixed-price, FOB destination and include any discounts for service contract with a base period of twelve months an additional four year option periods. All quotes submitted must be in conformance with the scope of work. Quotes must be valid for 60 days. See attached Scope of Work, Price Schedule and wage determination.
