This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Federal Acquisition Regulation (FAR) Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. B. The Great Plains Area Indian Health Service (IHS) intends to award one single award Firm-Fixed Price, Non-Personal Service, Commercial Item, Purchase Order in response to Request for Quote (RFQ) RFQ-20-126. Partial awards will not be made. The Government reserves the right to not make an award, if necessary. This is a RFQ only and in no way obligates the Government to award a Purchase Order or to reimburse offeror(s) for any costs incurred in preparing their quote. C. This solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2020-06, effective June 5, 2020. D. The RFQ is issued as unrestricted and the associated NACIS code is 561710. E. The quoted unit pricing must be all inclusive, to include but not be limited to shipping, travel, lodging, per diem, fringe benefits, federal, state and local taxes (contact the local Tribal TERO Office to see if taxes are applicable), plus all other costs pertinent to the performance of this purchase order. Delivery to: Indian Health Service (IHS), Standing Rock Service Unit, includes Fort Yates Hospital, Fort Yates, ND; Cannonball Health Station, Cannonball, ND; McLaughlin Health Center, McLaughlin, SD; Wakpala Health Station, Wakpala, SD; Bullhead Health Station, Bullhead, SD. (See attached Statement of Work) Period of Performance: August 1, 2020 – July 31, 2021 (Base Year), with Four (4), One (1) Year, Option Periods. H. FAR 52.212-1 Instructions to Offerors- Commercials Items (June 2020) – Applies to this acquisition and is incorporated by reference. Quotes shall be submitted on company letterhead stationery, signed and dated. Quotes shall include the following information to be considered technically acceptable: 1. Solicitation Number: RFQ-20-126 2. Closing Date: June 29, 2020 at 3:00 PM CST 3. Name, address, and telephone number of offeror and email address of contact person. 4. Technical description of the items being offered in sufficient detail to evaluate compliance with the requirements in the solicitation. This may include product literature, or other documents, if necessary. 5. Terms of any express warranty. 6. Price and any discount terms. 7. "Remit to" address, if different than mailing address. 8. Acknowledgement of solicitation amendments (if any issued). 9. A completed copy of the representations and certifications at FAR 52.212-3 (see FAR 52.212-3(b) for those representations and certifications that the offeror shall complete electronically). 10. A statement specifying the extent of agreement with all terms, conditions, and provisions included in the solicitation. Offerors that fail to furnish required representations or information, or reject the terms and conditions of the solicitation, may be excluded from consideration. 11. Completed Integrated Pest Management Services Price Schedule as attached to this RFQ. In addition to the above items, contractors shall provide the following in order to be considered technically acceptable: a. The Contractor MUST submit documentation showing they are a certified Pest Control Company licensed with the State of North Dakota or South Dakota to perform pest control services. Contractor must have a professional services license (structural) for household pest control. b. Contractor shall provide the name of three (3) references from commercial entities. c. Contractor shall provide a copy of licenses, bonds and insurance. Basis for Award: Lowest Price Technically Acceptable (LPTA). LPTA factors in determining the successful contractor: - Meet the Requirements and Contractor Qualification criteria in the Statement of Work as attached to this RFQ - Meet the Delivery Schedule I. The offeror shall complete only paragraph (b) of this provision if the offeror has completed the annual representations and certifications electronically via the System for Award Management (SAM) website accessed through https://www.sam.gov. If the offeror has not completed the annual representations and certifications electronically, the offeror shall complete only paragraphs (c) through (o) of this provision. J. In addition, it must contain a statement to the effect that it is firm for a period of at least 60 days from the date of receipt by the Government. Quoters will need a DUNS number, TIN number and be registered and have an active registration with the System for Award Management (SAM).
