The Indian Health Service (IHS), Crownpoint Healthcare Facility, located in/at NM State Hwy 371, Navajo Route 9, Crownpoint, NM 87313 is seeking capable sources to provide comprehensive medically necessary health care services inclusive of inpatient/outpatient services, with all ancillary/support services, emergency medical services, air transportation , ground ambulance, public health nursing, behavioral health, school based clinics, optometry, dental, and other necessary health care services to IHS beneficiaries residing within the following defined Navajo Chapter boundaries and portions of the communities: Counselor, Ojo Encino and Torreon Chapters; Star Lake, Cuba, Rincon Marcus, Tinian communities; and southwest of Lybrook School. (see attach Covered Communities Map) The anticipated period of performance is June 1, 2023 to May 31, 2024 with two options periods June 1, 2024 to May 31, 2025 and June 1, 2025 to May 31, 2026
