(i) This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. Brand Name or Equal. (ii) Solicitation number RFQ PHC6489 applies and is issued as a Request for Quote (RFQ). (iii) This solicitation document and incorporated provisions and clauses are those in effect through the Federal Acquisition System Circular No. 2005-95. (iv) Set aside for Small Business. (v) See attached CLINS listing that provides the description, salient characteristics and part numbers. All proposed equipment must meet the salient characteristics identified on the attached CLINS listing. This is a brand name of equal requirement. (vi) See attached CLINS listing for descriptions and related salient characteristics. (vii) Delivery and acceptance terms for this order FOB Destination. Deliveries will be made to Pinon Health Center, 30 days After Award. (viii) The provision at 52.212-1, Instructions to Offerors - Commercial Items (Oct. 2018) shall apply. (ix) 52.212-2 Evaluation - The award will be made to the lowest price technically acceptable offeror. To be considered for an award, the offeror shall provide evidence that their proposed product meets all listed salient characteristic identified. Only new equipment shall be quoted, no grey marketed or refurbished equipment should be quoted; all proposed equipment must meet the salient characteristics identified on the attached CLIN document. The total price derived by with the quantities all items listed in the attached CLINS listing will be used to determine the lowest price offeror. (x) Offerors shall include a completed copy of the provision at 52.212-3, Offeror Representations and Certifications - Commercial Items with its offer. This solicitation incorporates solicitation provisions and clauses by reference, what the same force and effect as if they were given in full text. Upon request, the Contracting Officer will make their full text available. The offeror is cautioned that the listed provisions may include blocks that must be completed by the offeror and submitted with its quotation or offer. In lieu of submitting the full text of those provisions, the offeror may identify the provision by paragraph identifier and provide the appropriate information with its quotation or offer.
