OBJECTIVE: The Contractor shall provide Ophthalmic Equipment Service and Maintenance for the following government owned property with the following specifications: Item # / Description of Service / Location / Unit of Issue 1. Keeler BIO, Haag Streit slit lamp and table, Welch Allyn ophthalmoscope & retinoscope & transilluminator, 4 fundus lenses, perkins applanation tonometer, San Felipe Health Center / 1 Contractor shall inspect, clean, lubricate and calibrate as indicated each item Contractor shall provide repair service on location or at a designated service center if needed Contractor shall provide a detailed report of services provided and an invoice for all charges. Contractor shall provide standard air shipping for return of repaired instrument Contractor shall provide technicians with industry standard training and qualifications including factory certification for the equipment serviced. Contractor shall have and maintain liability insurance. PERIOD OF PERFORMANCE Period of Performance will be for a Base + 4 Option Periods. HOURS OF OPERATION & PLACE OF DELIVERY: Hours of operation will be M-F from 8:00 AM to 4:30 PM. STATEMENT OF OBJECTIVES: Locate eligible and qualified Small Business Indian Firms to support IHS procurement needs under NAICS code: 811310 - Commercial and Industrial Machinery and Equipment (except Automotive and Electronic) Repair and Maintenance Utilize the information received with future acquisition planning efforts and potential Buy Indian set aside. HOW TO RESPOND: This is a sources sought notice under the authority of the Buy Indian Act. The Indian Health Service (IHS) is attempting to locate qualified and eligible Small Business Indian Firms, “Native American Indian Owned and Operated businesses,” that can provide services/supplies to the Indian Health Service. This is a request for information only, this is not a solicitation. Responses to this notice are not considered offers and cannot be accepted by the Government to form a binding contract.
