This solicitation notice is a request for proposals for a Non-Personal Service Contract for a Contractor to provide registered nurse services for the Ambulatory Care Department, Northern Navajo Medical Center, Highway 491 North, Shiprock, New Mexico, 87420 - Navajo Area Indian Health Service (IHS) throughout the duration of the contract period of performance. Services shall be performed in accordance with the attached Performance Work Statement (PWS). This Request for Quote (RFQ) is Unrestricted for a Non-Personal Service Contract per FAR 37.401(a) for RN services to be performed at the Northern Navajo Medical Center, Shiprock, New Mexico of the Indian Health Service (IHS). The tasks to be performed are cited in the attached Performance Work Statements. There are 2 nurse positions to be filled for Ambulatory Care and 1 position for OB-Gyn Inpatient The Period of Performance will be issued as Base plus Option Periods: Base Period: Date of Award through 6 months Option 1 Period: extend an additional 6 months SHIFT: Ambulatory Care: Monday - Friday 0800 am to 1630 OB-GYN: 12 hours rotational (day or night) including weekends and Federal Government holidays. The Government DOES NOT guarantee a specific shift to any individual(s). It will greatly depend on patient case load. The Government DOES NOT guarantee that the contractor will work the number of hours ordered. The hours ordered are only estimated. The assignment is through the stated period of performance or until the position is filled with a permanent hire. In such instances, the nurse will be released from contract assignment Education: Degree or diploma from a professional nursing program approved by the legally designated State accrediting agency at the time the program was completed by the application. Experience: The RN needs to have at least 2 years of clinical experience in professional nursing care principles, practice, and procedures. Must have knowledge of immunization, diabetes, a background in a clinic situation with a primary setting from newborn to elderly care with chronic medical problems and the ability to recognize signs and/or symptoms to quickly provide emergency care. Certifications and Licensure (MANDATORY): - must possess Register Nurse Licensure and maintain current State licensure while employed with Indian Health Service (IHS). - a Basic Life Support (BLS) Certification with American Heart Association for both positions Housing: No government housing available. There are motels, restaurants, mall, etc. in Farmington, NM, 29 miles East of Shiprock, NM and in Cortez, CO, 41 miles North of Shiprock, NM. REQUIREMENTS: Incomplete submission(s) of REQUIREMENTS WILL NOT be reviewed or considered including direct submission to the hospital unit managers. In addition, any profiles received before or after the RFQ dates WILL NOT be accepted. 1. Return attached fill in Pricing. Quote(s) submitted will be negotiated. The rate shall be an all-inclusive rate of travel, per diem, lodging/housing, transportation, meals, and applicable taxes (i.e. federal, state and local). 2. A complete and signed Certification Statement for Performance Work Statement (PWS) #08-01 Registered Nurse (RN). (See Attached) 3. Profile(s) must include valid phone number and email address ***ONLY SUBMIT INDIVIDUALS WHO ARE AVAILABLE DURING THE DELIVERY SCHEDULE * 4. Three (3) current references with active telephone numbers. The references MUST be noted in a separate Word or PDF document and submitted as an attachment. 5. Required to float to other units on an as needed basis. 6. Contractor will not start until the Purchase Order is issued - signed by the Contracting Officer 10. Certificate of Liability Insurance (COI) for each candidate also be considered, should one or more quotes represent the best value 7. Past Performance: Provide experience of comparable scope and complexity in providing services within the past three (3) years of operation The government reserves the right to issue a single award or multiple award to the Schedule contractor(s) whose quote(s) represents the best value as defined by FAR 2.101. In determining best value, the following factors will be considered: price, past performance, and qualification(s)/experience of each proposed Registered Nurse candidate.
