The Indian Health Service, Gallup Indian Medical Center, located at 1808 West Aztec Ave. Gallup, New Mexico 87301, has a continuous requirement for various medical supplies and will be awarding multi-Blanket Purchase Agreements (BPAs) to satisfy the need. The BPAs for medical supplies will be requiring immediate deliveries on an “As Needed Basis”. The period of performance will be for a base period of 12 months from the date of award with one option year to extend for an additional consecutive 12 months thereafter. The North American Industry Classification System or NAICS codes are 339113 Surgical Appliance and Supplies Manufacturing with a small business concern of 750 size standard, and 339112 Surgical and Medical Instrument Manufacturing with a small business concern of 1000 size standard. Any vendors interested are encouraged to submit their capability statement, Small Business priorities, authorized distributor letters, medical supply lists, price lists, and catalogs to Gallup Indian Medical Center – Acquisitions department via email to: Stefanie.atene@ihs.gov. In accordance with: 52.204-7(b)(1) System for Award Management an Offeror is required to be registered in SAM when submitting an offer or quotation, and shall continue to be registered until time of award, during performance, and through final payment of any contract, basic agreement, basic ordering agreement, or blanket purchasing agreement resulting from this solicitation. In accordance with 52.204-13(c) System for Award Management Maintenance, The Contractor shall maintain registration in SAM during contract performance and through final payment of any contract, basic agreement, basic ordering agreement, or blanket purchasing agreement. The Contractor is responsible for the currency, accuracy and completeness of the data within SAM, and for any liability resulting from the Government's reliance on inaccurate or incomplete data. To remain registered in SAM after the initial registration, the Contractor is required to review and update on an annual basis, from the date of initial registration or subsequent updates, its information in SAM to ensure it is current, accurate and complete. Updating information in SAM does not alter the terms and conditions of this contract and is not a substitute for a properly executed contractual document. 52.211-6(a) Brand Name or Equal - If an item in this solicitation is identified as "brand name or equal," the purchase description reflects the characteristics and level of quality that will satisfy the Government’s needs. The salient physical, functional, or performance characteristics that "equal" products must meet are specified in the solicitation.
