This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. BACKGROUND: The Navajo Area Indian Health Service, Kayenta Health Center (KHC) is requesting for offers for a contractor to deliver the following items in accordance with the attached specifications: (4 EACH) Wall Mount Eye Wash Station (1 EACH) Wall Mount Swing Down Eye Wash (5 EACH) Stainless Steel Cabinet (5 EACH) Mixing Valve SHIPPING ADDRESS: Kayenta Health Center HWY 160, South Mile Post 394.3 Kayenta, AZ86033 EVALUATION CRITERIA: This procurement is being conducted under FAR Part 13 Simplified Acquisition Procedures. The Government reserves the right to issue a firm-fixed price single award or multiple awards to the contractor(s) whose offer represents the Lowest Price Technically Acceptable (LPTA); award will be made on the basis of the lowest evaluated price of proposals meeting or exceeding the acceptability standards for non-cost factors. As used in this clause – Payment request means a bill, voucher, invoice, or request for contract financing payment with associated supporting documentation.
