This contract provides a comprehensive Therapeutic Shoe Program for patients with diabetes. The contractor will perform patient evaluations, measurements, fittings, dispensing, adjustments, modifications, replacements, and follow-up services. Supplies include therapeutic shoes, custom or prefabricated inserts, diabetic socks, and other medically necessary diabetic foot items. All footwear and supplies must meet CMS coverage requirements for Therapeutic Shoes for Persons with Diabetes and accepted industry standards.
