The consultant shall provide technical assistance to assess and improve the Case Management Program across three healthcare facilities: Northern Navajo Medical Center, Dzilth-Na-O-Dith-Hle Health Center, and Four Corners Regional Health Center. The Period of Performance for this requirement is November 1, 2025 to April 30, 2026. The North American Industry Classification System or NAICS code is 541611 Administrative Management and General Management Consulting Services with a small business size standard of $24.5 million. The Request for Quotations No. 75H71025Q00345 using the principles of FAR 12 Acquisition of Commercial Items in conjunction with FAR 13 Simplified Acquisition Procedures will be issued on or by September 8, 2025. Evaluation factors may include technical capability to meet the government need, ability to adhere to the period of performance schedule, past performance, and price. Quotes will be due on or by September 16, 2025. The acquisition is a 100% small business set aside.
