The following factors shall be used to evaluate offers: Lowest Price Technically Acceptable - Price - MEETS ALL REQUIREMENTS ACCORDING TO THE ATTACHED STATEMENT OF WORK Quarterly reports representing up to a minimum of three years of data for each chart abstracted measures as identified by the individual facility § Facility specific tabular reports and control charts with trended data for each measure the Facility submits § Comparative graphic and tabular reports, including year-to-date information, for each measure the Facility submits § The eCQM report denotes the scores for each selected eCQM measure for a specific time frame; similar to the CMS Performance Summary Report. § Data export reports are exportable from the website for chart abstracted and eCQM measures the facility submits Compliance with The Joint Commission (TJC) & Centers for Medicare and Medicaid Services (CMS) Criteria Have a database that provides CMS scoring per their specifications Database produces an QRDA file and upload our data directly to CMS. Have a 2015 CURES Certified EHR Are certified for all 13 eCQMS for CMS submission Meets section 2A on the Statement of Work VENDORS SUBMITTING OR EQUAL ITEMS MUST SUMBMIT DESCRIPTITVE LITERATURE SHOWING HOW THEIR PRODUCT MEETS OR EXCEEDS THE REQUIREMENTS BEING SOLICITED – INCLUDES SERVICE CONTRACT.
