Services shall be performed in accordance with the attached Performance Work Statement (PWS). The Period of Performance will be issued for 6 months 1) Base Period 2) with an Option 1 Period to extend an additional 6 months. The assignment is through the stated period of performance or until the position is filled with a permanent hire. In such instances, the nurse will be released from contract assignment. The North American Industry Classification System or NAICS code is 561320 with a small business size standard of $30M. This solicitation is using the principles of FAR 12 Acquisition of Commercial Items in conjunction with FAR 13 Simplified Acquisition Procedures. In accordance with 52.204-7(b)(1) prospective contractors must have – and maintain (in accordance with 52.204-13(c) – an active profile in the System for Award Management at beta.SAM.gov in order to submit a quote. EVALUATION CRITERIA: The Government reserves the right to issue a single award or multiple awards to any contractor(s) whose quote represents the best value as defined by FAR 2.101. In determining best value, Price and other Evaluation Factors will be considered: Past Performance and Qualifications. The Evaluation Factors and significant factors when combined are significantly more important than cost or price. The socio-economic status of an offer may also be considered, should one or more quotes represent the best value. All candidates are required to have an official United States social security number. SUBMITTAL REQUIREMENTS: Incomplete submission(s) of REQUIREMENTS WILL NOT be reviewed or considered including direct submission to the hospital unit managers. Hourly Rate shall be an All-Inclusive Hourly Rate, meaning travel, per diem, lodging/housing, transportation, meals, and applicable taxes (i.e. federal, state and local). Provide evidence of Provider’s Experience of comparable scope and complexity in providing services within the past three (3) years. Provide evidence of the Contract Company’s Past Performance in providing steady supply of personnel to fill the vacancies. Provide a completed IHS IEE Representation Form (attached).
