Responses to this sources sought notice will only be used to make appropriate acquisition decisions and to identify interested small businesses including: Service-Disabled (SDVOSB), Woman-Owned (WOSB/EDWOSB). The NAICS for this requirement is 811210 – Electronic and Precision Equipment Repair and Maintenance. This is a firm-fixed price requirement. The period of performance consists of a one base-year period from 10/01/2024 to 09/30/2025 and four one-year option periods. The Phoenix Area Indian Health Service (PAIHS) has a need for Preventive Maintenance & Repair Services for two (2) GE Carestation Anesthesia Machines at the following location: Whiteriver Indian Health Service Hospital 400 West Apache Drive Whiteriver, Arizona 85941 The contractor shall provide all supervision, labor, parts, materials, tools, supplies, maintenance, calibration, ground transportation, and equipment service for the two General Electric Carestation Anesthesia Machines. The Contractor shall repair or replace all components necessary to return units to Original Equipment Manufacturer’s specifications. The Contractor shall warrant all services performed and units shall remain defect free 30 days from the date of service/maintenance/repair acceptance. Interested parties are encouraged to respond to this notice no later than August 21, 2024 at 1:00pm MST. Responses shall include: (1) Company Name & Address (2) Point of Contact and Contact Info (3) Contractor’s Unique Entity ID (SAM) number (4) Type of small business (5) Any current contracts that the contractor is able to provide the requested services under either GSA or Federal Supply Schedules, or other GWACs (6) Statement indicating how many calendar days are estimated it would take to setup for PM Services to the GE Carestation Anesthesia Machines.
