This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. **THIS IS A BUY INDIAN SET ASIDE per Buy Indian Act, 25 U.S.C. 47 and only offers form Buy Indian small businesses will be accepted. The Navajo Area Indian Health Service, Pinon Health Center is requesting for offers for a contractor to provide Preventive Maintenance Services for (4) Air Conditioning Units, (2) Chiller Compressor Units, and (1) Split System Unit. Services shall be performed in accordance with the attached Statement of Work (SOW). PERIOD OF PERFORMANCE: 05/1/2024 – 04/30/2025 EVALUATION CRITERIA: The Government reserves the right to issue a single award or multiple awards to contractor(s) whose quote represents the best value as defined by FAR 2.101. In determining best value, Price and other Evaluation Factors will be considered: Past Performance, Experience, Qualifications, and Logistics. The Evaluation Factors and significant sub factors when combined are significantly more important than cost or price. The socio-economic status of an offer may also be considered, should one or more quotes represent the best value. Past Performance: Provide a letter of reference from a past customer. Qualifications: Provide evidence that you are qualified to do the work. Logistics: Provide a plan of action describing the logistics of how you will be providing services. Provide a completed IHS IEE Representation Form (attached). Clauses and Provisions: 52.222-41 Service Contract Labor Standards (Aug 2018) Contractor shall meet or exceed the minimum required wage determination as stated in the U.S. Department of Labor - Wage Determination No.: 2015-5477 52.217-8 Option to Extend Services (Nov 1999) The Government may require continued performance of any services within the limits and at the rates specified in the contract. These rates may be adjusted only as a result of revisions to prevailing labor rates provided by the Secretary of Labor. The option provision may be exercised more than once, but the total extension of performance hereunder shall not exceed 6 months. (Dec 2015) (a) Public Law 103-227, Title X, Part C, also known as the Pro-Children Act of 1994 (Act), 20 U.S.C. 7183, imposes restrictions on smoking in facilities where certain federally funded children's services are provided. The Act prohibits smoking within any indoor facility (or portion thereof), whether owned, leased, or contracted for, that is used for the routine or regular provision of: (i) kindergarten, elementary, or secondary education or library services or (ii) health or day care services that are provided to children under the age of 18. The statutory prohibition also applies to indoor facilities that are constructed, operated, or maintained with Federal funds. (b) By acceptance of this contract or order, the Contractor agrees to comply with the requirements of the Act. The Act also applies to all subcontracts awarded under this contract for the specified children's services. Accordingly, the Contractor shall ensure that each of its employees, and any subcontractor staff, is made aware of, understands, and complies with the provisions of the Act. Failure to comply with the Act may result in the imposition of a civil monetary penalty in an amount not to exceed $1,000 for each violation and/or the imposition of an administrative compliance order on the responsible entity. This Sources Sought Notice is for the Navajo Area Indian Health Service, Pinon Health Center issued in accordance with FAR 5.101. The purpose of this notice is to identify potential sources for providing preventive maintenance services on (4) rooftop air conditioning units, (2) chiller compressor units, and (1) split system unit at the Pinon Health Center. This notice does not commit the Government to issue a solicitation or make an award OR to prelude a solicitation expected to be issued from closure of notice. Pinon Health Center is Associated with Chinle Service Unit - Indian Health Service in the Heart of the Navajo Reservation - Northern Az. Pinon Health Center provides care to approximately 11,000 patients - the majority of which are Navajo and many are traditional Navajo who still speak the language. A collegial group of providers offer services in primary care, women's health, counseling, dental, dietary and community nutrition, health promotions, lab, x-ray, native medicine, optometry, pharmacy, physical therapy, speech therapy, podiatry and public health nursing. When not providing care in the clinic our group enjoys book clubs, potlucks, gardening, fire pits, learning about the Navajo culture, hiking, rock climbing, camping, biking, cross country skiing. Downhill skiing, kayaking rock climbing, fishing and national parks are all within a few hours away. Description of Services: The Contractor shall provide preventive maintenance services on (4) rooftop air conditioning units, (2) chiller compressor units, and (1) split system unit. Duration of Requirement: Date of Award to December 31, 2024 Place of Performance: Pinon Health Center, Facility Maintenance Department, 2175 State Road 4, Pinon, AZ 86510 Instructions to Industry: All capable parties are encouraged to respond. Responses must directly demonstrate the company’s capability, experience, and ability to marshal resources to effectively and efficiently perform the objectives described above. Generic capability statements are not sufficient and will not be considered compliant with the requirements of this notice. Capability Statement: Detailed capability statement addressing the company’s qualifications and ability to provide the requirements listed herein, with appropriate and specific documentation supporting claims of recent organizational and staff capability to support this requirement. If significant subcontracting or teaming is anticipated in order to deliver technical capability, organizations should address the administrative and management structure of such arrangements. Experience: Provide a list of 3-5 private industry or Government contracts for the same services that you have performed within the last 3 years. For each contract, include the company’s Point of Contact’s name, email address, telephone number, dollar value of contract, and description of the services provided on the contract. The Government may contact these entities to conduct past performance checks. If American Indian/Native American owned small business, then complete attached IEE Representation form. The Government will evaluate market information to ascertain potential market capacity to: 1) provide services consistent in scope and scale with those described in this notice and otherwise anticipated; 2) secure and apply the full range of corporate financial, human capital, and technical resources required to successfully perform similar requirements; and 3) implement a successful project management plan that includes: compliant with tight program schedules; cost containment: meeting and tracking performance; hiring and retention of key personnel and risk mitigation.
