The Indian Health Service, Crownpoint Health Care Facility, Inscription House Health Center, Dzilth-na-o-dith-hle Health Center, and Northern Navajo Medical Center are in need of an Optometrist who will provide on-site services to eligible beneficiaries. Each facility needs one (1) Non-Personal Service Contract to fulfill its optometry service needs. The North American Industry Classification System or NAICS code is 621320 Office of Optometrists with a small business size standard of $9.0 million. The Request for Quotations No. 75H71024Q00099 using the principles of FAR 12 Acquisition of Commercial Items in conjunction with FAR 13.5 Simplified Procedures for Certain Commercial Products and Commercial Services will be issued on or by June 26, 2024. Evaluation factors may include key personnel are required to meet the minimum requirements stated in the performance work statement, past performance, and price. Quotes will be due on or by July 25, 2024, by 2:30 p.m. MDT. The acquisition is a total 100% Small Business set aside.
