The Indian Health Service, Gallup Indian Medical Center, located at 1808 West Aztec Ave. Gallup, New Mexico 87301, has a Request for Quotation (RFQ) for a requirement of Janitorial Supplies to be delivered on or before August 31, 2023. The North American Industry Classification System or NAICS codes are 325611 Soap and Other Detergent Manufacturer with a small business concern of 1100 size standard. In accordance with: 52.204-7(b)(1) System for Award Management an Offeror is required to be registered in SAM when submitting an offer or quotation, and shall continue to be registered until time of award, during performance, and through final payment of any contract, basic agreement, basic ordering agreement, or blanket purchasing agreement resulting from this solicitation. In accordance with 52.204-13(c) System for Award Management Maintenance, The Contractor shall maintain registration in SAM during contract performance and through final payment of any contract, basic agreement, basic ordering agreement, or blanket purchasing agreement. The Contractor is responsible for the currency, accuracy and completeness of the data within SAM, and for any liability resulting from the Government's reliance on inaccurate or incomplete data. To remain registered in SAM after the initial registration, the Contractor is required to review and update on an annual basis, from the date of initial registration or subsequent updates, its information in SAM to ensure it is current, accurate and complete. Updating information in SAM does not alter the terms and conditions of this contract and is not a substitute for a properly executed contractual document. 52.211-6(a) Brand Name or Equal - If an item in this solicitation is identified as "brand name or equal," the purchase description reflects the characteristics and level of quality that will satisfy the Government’s needs. The salient physical, functional, or performance characteristics that "equal" products must meet are specified in the solicitation. Evaluation and award will be based upon the lowest price meeting the specifications. The price list is attached and has listed supplies by Brand Name Manufacture ID numbers and includes or Equal to Brand opportunity. The Or Equal to Brand specifications shall be submitted with the price listing that match or exceed the specified brand item. This RFQ is 100% Indian Owned set aside. In accordance with the Buy Indian Act, 25 U.S.C. 47, the Indian Health Service shall give preference at all times, as far as practicable, to Indian economic enterprises.
