REQUEST FOR QUOTE NOTICE ****************************************************************************** BACKGROUND: The Indian Health Service (IHS), Inscription House Health Center, located in Tonalea, Arizona 86033 is seeking capable sources to provide Universal Power Supply for Laboratory equipment. The applicable North American Industry Classification System (NAICS) code assigned to this procurement is 335999 – All Other Miscellaneous Electrical Equipment and Component Manufacturing with a business size standard of 500 employees. In accordance with the Buy Indian Act, 25 U.S.C. 47, the Indian Health Service shall give preference at all times, as far as practicable, to Indian economic enterprises. If your firm is capable of providing the supplies and/or services described in this notice, complete the attached representation form and submit it along with the rest of the requested documents identified in this notice. EVALUATION CRITERIA: This procurement is being conducted under FAR Part 13 Simplified Acquisition Procedures. The Government reserves the right to issue a firm-fixed price single award or multiple awards to the contractor(s) whose offer represents the Lowest Price Technically Acceptable (LPTA); award will be made on the basis of the lowest evaluated price of proposals meeting or exceeding the acceptability standards for non-cost factors. Quotes will be due on or before the closing date of the solicitation. As used in this clause – Payment request means a bill, voucher, invoice, or request for contract financing payment with associated supporting documentation.
