The Indian Health Service, Shiprock Service Unit, Dzilth-Na-O-Dith-Hle Health Center, located in Bloomfield, New Mexico, has an upcoming requirement for the purchase seven (7) Eyewash Stations and installation of the seven (7) Eyewash Stations. The anticipated period of performance is yet to be determined, not to exceed 12 months. A site visit will be conducted at the Dzilth-Na-O-Dith-Hle Health Center (DZHC), 6 Road 7586, Bloomfield, NM 87413, on March 16, 2023 at 10:00 am (MST). Attendance of this site visit is encouraged but not required. All costs incurred of any kind for the offeror to attend the site visit shall be paid by the offeror only. The government is not responsible for any proposal costs or cost associated with the voluntary site visit. Potential Offerors must submit names of all attendees (not to exceed two per company) to the following points of contact: Ernest Sandoval, email: ernest.sandoval@ihs.gov and Juliana Jim-Johnson, email: Juliana.jim-johnson@ihs.gov, no later than March 13, 2023 at 3:00 pm (MST). *This information must be provided in advance in order to ensure access to the facility. Evaluation factors may include technical capability to meet the government need outlined in the Statement of Work/Commercial Specifications, professional certifications, past performance, and price. Questions may be submitted in writing via email no later than March 22, 2023. Quotes will be due on or before March 28, 2023. The Acquisition is set aside as Unrestricted. In accordance with 52.204-7(b)(1) prospective contractors must have – and maintain (in accordance with 52.204-13(c) – an active profile in the System for Award Management at beta.SAM.gov in order to submit a quote. *There is no SF 1449 attached as the total estimated value is under the Simplified Acquisition Threshold.
