THIS IS A BUY INDIAN SOURCES SOUGHT NOTICE This Sources Sought Notice is for the Navajo Area Indian Health Service, Four Corners Regional Health Center issued in accordance with FAR 5.101 for Richmond Gas Water Heaters. The purpose of this notice is to identify potential sources who are able to provide the identified products needed by Four Corners Regional Health Center to help meet agency’s identified requirements. This sources sought notification does not commit the Government to issue a solicitation or make an award OR to prelude a solicitation expected to be issued from closure of notice. The Four Corners Regional Health Center (FCRHC) is located in Red Mesa, Arizona near the borders of Utah, Colorado and New Mexico. FCRHC opened in January 2007 and is under the administrative oversight of Shiprock Service Unit. FCRHC provides ambulatory care, sub-acute care and community health services in the following areas: family medicine, internal medicine, optometry, dental, pharmacy, radiology, physical therapy, laboratory, social services, case management, mental health, public health nursing; diabetes education, health promotion and disease prevention; and traditional medicine. Description of Product: Gas Water Heaters Quantity: 24 each Delivery Timeframe: Within 45 days Place of Delivery: Four Corners Regional Health Center, US Highway 160, Navajo Route 35, Red Mesa, Arizona (86514) In accordance with the Buy Indian Act, 25 U.S.C. 47, the Indian Health Service shall give preference at all times, as far as practicable, to Indian economic enterprises. If your firm is capable of providing the supplies and/or services described in this notice, complete the attached representation form and submit it along with the rest of the requested documents identified in this notice. This solicitation is issued as a BUY INDIAN Set-Aside, per Buy Indian Act, 25 U.S.C. 47, only offers from a Buy Indian Small Business will be accepted. The Government requests interested parties submit a capability package to this notice which includes: Company Name. Company DUNS number. Company point of contact, mailing address, and telephone and fax number(s), and website address Name, telephone number, and e-mail address of a company point of contact who has the authority and knowledge to clarify responses with government representatives.. System for Award Management (SAM) registration status. All respondents must register on the SAM located at http://www.sam.gov/. References: Provide a list of all private industry or government contracts for similar services that you have performed within the last 3 years. Please include the customers’ contact names, addresses, telephone number, dollar value of contract, and brief description of the services provided on the contract. After a review of the responses received, a pre-solicitation synopsis and solicitation may be published on a government GPE.
