Surveys are to be done annually and after installation of new Radiology equipment and the replacement of Mammography unit x-ray tube. The requirement will include a base period plus four 12-month option periods. Base Period: April 1, 2022 – March 31, 2023 1st Option Period: April 1, 2023 – March 31, 2024 2nd Option Period: April 1, 2024 – March 31, 2025 3rd Option Period: April 1, 2025 – March 31, 2026 4th Option Period: April 1, 2026 – March 31, 2027 The North American Industry Classification System or NAICS code is 541690 with a small business size standard of $16.5 million. The Request for Quotations No. 75H71022Q00022 using the principles of FAR 12 Acquisition of Commercial Items in conjunction with FAR 13 Simplified Acquisition Procedures will be issued on or by March 17, 2022. Evaluation factors may include technical capability to meet the government need, professional certifications, past performance, and price. Quotes will be due on or by March 24, 2022. The acquisition is a total small business set-aside.
