This is a solicitation for commercial items prepared in accordance with the format in subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation where proposals are being requested and a written solicitation will not be issued. This solicitation notice is a request for proposals for Annual Preventative Maintenance of Generators and Load Bank Services for the Gallup Indian Medical Center located in Gallup New Mexico throughout the duration of the contract period of performance of a base plus four option years. Period of Performance: Base Year: November 1, 2021 – October 31, 2022 Option Year One (1): November 1, 2022 – October 31, 2023 Option Year Two (2); November 1, 2023 – October 31, 2024 Option Year Three (3): November 1, 2024 – October 31, 2025 Option Year Four (4): November 1, 2025 – October 31, 2026 Statement of Work: Services shall be performed in accordance with the attached Scope of Work Statement (SOW). Price Schedule: Please submit proposals with the attached price schedule in order to be reviewed and evaluated. Attach additional information if necessary. Evaluation Criteria: The Government reserves the right to issue a single award or multiple awards to contractor(s) whose quote represents the best value as defined by FAR 2.101. In determining best value, Price and other Evaluation Factors will be considered: Past Performance, Experience, and Qualifications of the Contractor(s). This acquisition is a Total Small Business set aside. In accordance with 52.204-7(b)(1) prospective contractors must have - and maintain in accordance with 52.204-13(c) - an active profile in the System for Award Management at beta.SAM.gov in order to submit a quote.
