General requirements: The purpose of this notice is to obtain information regarding the availability and capability of qualified sources. The Navajo Area Indian Health Service (NAIHS), Health Promotion Disease Prevention Domestic Violence Prevention Initiative Project, seeks sources who can provide domestic violence and advocacy outreach services. Project services include providing effective, engaging, and coordinated virtual outreach services about domestic violence and sexual assault prevention education and advocacy training for young adults, adults, and elders in various settings, such as youth centers, detention centers, chapter houses, and senior citizen centers in Gallup Service Unit. The capability statement should include the following: Contractor qualifications and experience in domestic violence and advocacy outreach services. Contractor shall demonstrate how the company can provide the requested services that are required. Past performance in providing the requested services to the Indian Health Service, other Government (Federal or State) agency, or for a private facility, within the past three (3) years. Include contract number, point of contact, telephone number or e-mail address. The company DUNS number and System for Award Management (SAM) registration status. Government Point of Contact.
