General requirements: The purpose of this notice is to obtain information regarding the availability and capability of qualified sources. The Navajo Area Indian Health Service (NAIHS), Health Promotion Disease Prevention, Special Diabetes Prevention Program for the Indians Project, seeks sources who plan and implement wellness program for the Shiprock Service Unit. Project services include: Contractor will plan, develop and implement Teen Parent Educator to provide support for teen parents ensuring that they pursue pre-natal care with IHS, as well as providing early health and wellness intervention for infants of teen parents. Contractor will provide direct consultation in the adaptation of health services for Impact Concussion equipment training for Central Consolidated School District. Contractor will provide wellness consultation and access to exercise equipment and swimming pool at Natatorium for all Shiprock IHS patients. Contractor will provide support in community based HPDP promotional events related to wellness initiative and other HPDP related projects. Contractor will provide support in mental health wellness initiatives. Contractor will develop school and community early response to heart disease programs that will include Automated External Defibrillators (AED) and staff and community training at school sites. Maintain emergency response equipment for first aid kits, AED equipment, and EPI pens for schools and community. Contractor will provide Crisis Prevention Training for staff to de-escalate inappropriate student behaviors. Contractor will provide Health training and supplies for students, parents, community and staff. The capability statement should include the following: Contractor qualifications and experience in coordinating a wellness program. Contractor shall demonstrate how the company can provide the requested services that are required. Past performance in providing the requested services to the Indian Health Service, other Government (Federal or State) agency, or for a private facility, within the past three (3) years.
