The Whiteriver Indian Health Service Hospital requires a contracted project for the procurement and installation of 18 desks in the Family Care Unit Medical Staff Room for staff being relocated from the modular trailers that are scheduled for demolition. The scope of work detailed below outlines the required project outcomes including installation. The Contractor shall be responsible for all necessary work to achieve required outcomes including labor, equipment, tools, Infection Control and Risk Assessment (ICRA) policy adherence, incidental design, travel, per diem, TERO fees, and other related expenses required to complete the project. (HON: Brand Name or equal) The contract shall provide for the procurement and installation of 18 individual desks to match the existing desks Each desk shall include a 31” LED light with power supply. The contractor shall provide fully functional desk The contractor shall provide 18 desks. Each shall include: (18 ea) 24.00” deep x 36.00” wide work surface. The work surface shall be finished with standard laminates in Lowell Ash color. The edge shall be Lowell Ash color. (HON part# HLSLR2436.LLA1DL.GP) All legs/supports shall be Champagne Metallic color. (36 ea - HON Part# HLSL2428O.T4) and (18 ea HON part#HLSL65OS.T4) 14.25” deep x 36.00” wide x 13.00” height cabinets (18 ea - HON part# HLSL1436D.LLA1.LLA1.X) 31” LED lighting with power supply (18 ea - HON part#.HLED31AS) Installation The contractor shall assemble the desks onsite at the specified location. Packing materials and other materials that are not procured in this SOW shall be removed from the site with the contractor installing the desks. The period of performance of this contract is 60 calendar days after award. Facility day time hours of operation are 7:00 AM – 5:00 PM, Monday through Friday (excluding Federal holidays). Work outside of these hours must be requested in writing to the Contracting Officers Representative (COR). 100% set-aside for Native-American Owned Small Business.
