This solicitation is for a Non-Personal Service Performance Base Contract for Two (2) Registered Nurses (RN) "ON AS NEEDED BASIS" in the Operating Room Unit at Northern Navajo Medical Center (NNMC) located in Shiprock, NM. The tasks to be performed shall be in accordance with the attached Performance Work Statement (PWS). Period of Performance: Date of award for 6 months with possible extension. The assignment are through the stated period of performance or until the position is filled with a permanent hire. In such instances, the provider will be released from the contract WORK SHIFT: The shift hours are 8 hours from 0730-1600, Monday - Friday. The call starts at 1600-0730 for weekdays, and weekends start on Saturday at 0730 to Monday 0730. Must be able to take a call & respond within 15 minutes once they are notified of a surgical case. The Government DOES NOT guarantee a specific shift to any individual(s). It will greatly depend on patient case load. The Supervisor at IHS shall determine specific tour of duty. The Government DOES NOT guarantee that the contractor will work the number of hours ordered. The hours ordered are only estimated. HOUSING: No government housing available. . The nearby towns are Farmington, NM , 29 miles East of Shiprock, NM and in Cortez, CO, 41 miles North of Shiprock, NM. There are hotels, restaurants, mall, etc. in the two towns CONDITION OF EMPLOYMENT: Type of Citizenship: U.S. Citizenship is required. INITIAL APPOINTMENTS REQUIREMENTS: Education: Degree or diploma from a professional nursing program approved by the legally designated State accrediting agency at the time the program was completed by the applicant. Agency's Past Performance Number of similar contracts Timeliness of providing candidate(s) Registered Nurse Certification Requirements (Refer to PWS 7.4) - a valid and unrestricted State Nursing License - a current Basic Life Support (BLS) Certification with American Heart Association - a current Advance Cardiac Life Support (ACLS) Certification with American Heart Association - Pediatric Advance Life Support (PALS) Certification with American Heart Association Registered Nurse Availability - Upon fingerprinting clearance, contractor shall be available as soon as possible to start. - Registered Nurse shall be flexible to allow for short term (13-weeks), long term (1-year) and, where feasible, include intermittent support to meet unplanned needs that develop on a frequent basis. - The contractor shall rotate into other duty sections as needed to support patient care Professional Nursing Skills: 2 years or more OR experience; ASPAN member a plus - Associate RN Degree, Minimum - Experience for General Surgery, ENT, Ortho, Oral Surgery, OB/GYN, Orthopedic, Podiatry, Pedi Dental and Endoscopy; scrubbing role skills a plus. - Familiar with the AORNs standards, Infection Control, Safety and Joint Commission standards. - Ability to function for a schedule surgical and emergency procedure. - Ability to work with Electronic Health Record & Barcode Medication Administration Scanners - Able to float to other departments upon completion of a float competency skills checklist. - Ability & knowledge to learn the different types of equipment. Quotation in the line items above. The Government will consider competitive rates upon negotiation. Hourly Rate shall be an All-Inclusive Hourly Rate, Meaning travel, per diem, lodging/housing, transportation, meals, and applicable taxes (i.e. federal, state and local). The Attach Wage Determination Service Contract Act applies. 2. Past Performance: Provide experience of comparable scope and complexity in providing services within the past three (3) years of operation. 3. Qualifications: Provide complete candidate Profile (Curriculum vitae, updated resume, licensures / certifications, currently/valid contact information, Professional Nursing Skills and all relative documents for proposed candidate. Refer to PWS Section 5.0 & 7.0 4. Availability of candidate(s). 5. The Contractor will start until the Task Order is signed by Contracting Officer NOTE: Incomplete submission(s) of REQUIREMENTS WILL NOT be reviewed or considered including direct submission to the hospital unit managers. In addition, any profiles received before or after the RFQ dates WILL NOT be accepted. QUALIFICATION / EVALUATION: The Government reserves the right to issue a single award or multiple awards to any contractor(s) whose quote represents the best value as defined by FAR 2.101. In determining best value, Price and other Evaluation Factors will be considered: Past Performance and Qualifications. The Evaluation Factors and significant factors when combined are significantly more important than cost or price. As part of the evaluation process, candidates shall be interviewed to ensure their understanding of the PWS and to verify their qualifications to perform required services. The socio-economic status of an offer may also be considered, should one or more quotes represent the best value. All candidates are required to have an official United States social security number.
