This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. The Navajo Area Indian Health Services, Crownpoint Healthcare Facility (CPSU) Facility Management Department is requesting for submittal of offers for a contractor to provide Services ON BELOW ITEMS: 1. Preventative Maintenance Services for Ultrasound units for Radiology and OB Department. This acquisition will utilize the ordering procedures in accordance with FAR 8.405-1. Open Market item pricing will not be accepted. EVALUATION FACTORS: The Government reserves the right to issue a single award or multiple awards to schedule contractor(s) whose quote represents best value as defined by FAR 2.101. In determining best value, the Government reserves the right to select the most advantageous offer to the Government by evaluating and comparing initial quotations. The evaluation factors and significant subfactors when combined are approximately equal to cost or price. The socioeconomic status of an offeror shall also be considered, should one or more quotes represent best value. Factor 1: Technical Capabilities: Provide evidence that offered product meets the technical capabilities of attach salient characteristics of product. Subfactor: Special features of the equipment required for effective program performance. To be eligible for award offeror must be registered with the System for Award Management (SAM) (https://www.sam.gov) at the time of award, and remain active through payment. To be considered for award, your offer shall include the following documents/information: 1) Provide a Formal Quotation. The Government will consider competitive rates upon negotiation. 2) Specifications: Provide your product Specifications. Provide evidence that your offered product meets the attached Specifications. 3) Warranty consideration Important Note: If offer do not have above submittal requirements, your offer will be considered as nonresponsive.
