This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. The solicitation number and a statement that the solicitation is issued as a Request for Quotation (RFQ). The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2020-02 and 03 thru December 23, 2019. This requirement is set-aside to Women-Owned Small Business Concern. The designated NAICS code is 811219, with a size standard of $20.50M. Responses shall reference this combine synopsis-solicitation announcement number, “IHS1397567 Solicitation Preventive Maintenance and Repair for the Fume Hoods, PIMC.” See attached Price Schedule to be completed by the offeror. Provide quotation in accordance with the attached Scope of Work. This requirement is for Phoenix Indian Medical Center, Phoenix, Arizona for Preventive Maintenance and Repair for the Fume Hoods. The estimated period of performance is from April 8, 2020 to April 7, 2025. Tthe provision at 52.212-1, Instructions to Offerors-Commercial, applies to this acquisition and a statement regarding any addenda to the provision. The provision at 52.212-2, Evaluation-Commercial Items is applied and the specific evaluation criteria to be included in paragraph (a) of that provision.
