RFQ is modified to advise prospective offerors to submit offers via email to: Tochukwu.Aniche@ihs.gov. A previously attached document advised offers be submitted via GSA eBuy which does not apply to this requirement. RFQ closing date has also been extended thru Tuesday, Jan 21, 2020. Cardiology Telemetry Services (Event and Holter Monitors) to facilities and patients of the Phoenix Indian Medical Center (PIMC), in Phoenix, Arizona. Holter and/or event monitor are electronic devices that monitor the patient’s heart rhythm. The devices are provided to patients (in Phoenix and Northern Arizona areas) to diagnose cardiac arrhythmias. The Holter monitor is a 24-48 hour device and the Event monitor is a 30 day device. These devices monitor and allow transmission of the patient’s heart rhythm to the physician. The physician reviews the data and creates an official report. The company will provide notification to the physician based on pre- defined criteria. Please see attached SOW for details. Please include pricig for base as well as option years in proposal.
