INTENT TO SOLE SOURCE Navajo Area Indian Health Services, Gallup Indian Medical Center, Gallup NM intends to award a single source purchase order to Carestream Health Inc. 150 Verona St. Rochester, NY 14608-1733, under the authority of FAR Par 6.302-1 – Only one responsible source and no other supplies or services will satisfy agency requirements. NAICS code 811219 Other Electronic and Precision Equipment Repair and Maintenance. Period of performance shall be 01/01/2021 to 12/31/2021. This requirement provides for Support Services and Preventative Maintenance Radiology Equipment - Max CR Direct View system for the Gallup Indian Medical Center.
