This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Federal Acquisition Regulations (FAR) Subpart 12.6, as supplemented with additional information included in this notice. FAR Part 12, Acquisition of Commercial Items (Title VIII of the Federal Acquisition Streamlining Act of 1994) (P.L. 103-355). This announcement constitutes the only solicitation; bids are being requested and a written solicitation will not be issued. b. The (IHS) Cheyenne River Health Center intends to award a Firm Fixed-Price, Non-Personal Service Type, Commercial Item, Purchase Order in response to Request for Quote(RFQ) CRHC20-31. Partial award will not be made. The Government reserves the right to not make an award, if necessary. This is an RFQ only and in no way obligates the Government to award a Purchase Order or to reimburse offeror(s) for any costs incurred in preparing their quote. (ii) This Request for Quotation (RFQ) is being issued via solicitation number CRHC20-31. (iii) The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2020-07, dated 07/02/2020 (iv) This RFQ is issued as a 100% Small Business Set Aside and the associated NAICS code is 561710, with a small business size standard of $12.0 million. (v) Services purchased through this Purchase Order will be Pest and Rodent Management Services. A list of some, but not all, items, quantities/units of measure, and average yearly qtys, are attached. The “Unit Price” is an all-inclusive cost. All “inclusive cost” is defined to include, but not limited to travel, lodging, per diem, benefits, fuel, freight, TERO fees(contact local TERO Office to see if taxes area applicable), etc., plus all other costs pertinent to the performance of this contract. The period of performance will be a base year, plus the option to exercise, 4-one year option terms. (vi) Description of Supplies: Provide Pest and Rodent Management Services, at least quarterly, to the Main Hospital Facility, the Maintenance Shop, and the Government Housing area(Good River Community), which has an approximately 133 units, ranging from 1 bed room to 3 bedroom units, for the Maintenance Department, at the Cheyenne River Health Center located in Eagle Butte, South Dakota. Services to include but not limited to: Scheduling, Call Back Requirements, Residual Treatment, Restrictions, Rodent Removal, Disposal, Reporting, & Inspections, as per the attached Scope of Work. (vii) The period of performance will include one base year, plus the option to exercise four (4) option years. Delivery and acceptance terms for this order are FOB Designation. Delivery address: Cheyenne River Health Center, 24276 166th Street, Airport Road, Eagle Butte, South Dakota 57625. POP: Base Yr: 9/1/2020 – 8/31/2021, OY1: 9/1/2021 – 8/31/2022, OY2: 9/1/2022 – 8/31/2023, OY3: 9/1/2023 – 8/31/2024 & OY4: 9/1/2024 – 8/31/2025. Technical description of the item/service being offered in sufficient detail to evaluate compliance with the requirements in the solicitation. 5. Terms of any express warranty 6. Acknowledgement of Solicitation Amendments, if issued 10. A statement specifying the extent of agreement with all terms, conditions, and provisions included in the solicitation. Offers that fail to furnish required representation or information, or reject the terms and conditions of the solicitation may be excluded from consideration 11. Complete & signed SF18, Request For Quotations. In addition to the contractors shall provide the following in order to be considered technically acceptable: (a) The contractor MUST submit documentation showing they are a certified Pest Control Company licensed with the State of South Dakota to perform pest control services. Contractor must have a professional services license (structural) for household pest control. (b) Contractor shall provide the name of three (3) references from commercial entities, to include contact information. (c) Contractor shall provide a copy of licenses, bonds and insurance. (ix) Basis for Award: Lowest Price Technically Acceptable (LPTA). LPTA factors in determining the successful contractor will be the ability to meet the qualifications and scheduling of the requirement, and past performance. (x) Offerors must include a completed copy of the provision at 52.212-3, Offeror Representations and Certifications -- Commercial Items (Mar 2020), with its offer. The offeror shall complete only paragraphs (b) of the provision if the Offeror has completed the annual representations and certification electronically via the System for Award Management (SAM) Web site located at http://www.sam.gov (xi) The clause at 52.212-4, Contract Terms and Conditions -- Commercial Items (Oct 2018), applies to this acquisition.
