This amendment is to remove reference to Standard Form 1449 in section L.9.1; l.9.1 is revised to read as follows: L.9.1 Offerors must complete and include a copy of the combination synopsis/solicitation and indicate acceptance with all the terms and conditions contained in solicitation Sections A through M. In addition, the proposal must contain a statement to the effect that it is firm for a period of at least 120 days from the date of receipt thereof by the Government. This amendment is to remove reference to SF18 in Section L.4; submission of offers is hereby revised to read as follows: Offers must be submitted no later than 5 Oct 2021 by 4:00 pm ET. The Offeror shall submit one (1) electronic version of this solicitation, along with any exception to the terms and conditions or assumptions made in pricing the line items in a cover page to the Government. The proposal shall be delivered via email to the Contracting Officer at the following address: Debbie.Hope@psc.hhs.gov This is a combined synopsis/solicitation for the acquisition of commercial items prepared in accordance with the format in FAR Subpart 12.6, as supplemented with additional information included in this announcement. This announcement constitutes the only solicitation; quotes are being requested and a written solicitation will not be issued. This requirement is in support of the COVID-19 pandemic and other public health activities. This requirement is issued by the Dept. of Health and Human Services (DHHS), Program Support Center (PSC) in support of the Office of the Assistant Secretary for Health (OASH); Commissioned Corps Headquarters (CCHQ) to obtain contractor services to conduct rapid medical and dental examinations to Commissioned Corp applicants, and to perform pre- and post-deployment health assessments on PHS officers.
