The purpose of Solicitation Amendment Number 01 is to: (1) distribute Solicitation questions and answers, (2) amend Solicitation Attachment 01 - Statement of Work (2023-08-22), and (3) amend Solicitation Attachment 05 - Quote Instructions and Evaluation (2023-08-22). (i) This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6 as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotations are being requested and a written solicitation will not be issued (ii) The solicitation number is 75N95023Q00613 and the solicitation is issued as a request for quotation (RFQ). This acquisition is for a commercial product or service and is conducted under the authority of the Federal Acquisition Regulation (FAR) Part 13—Simplified Acquisition Procedures; FAR Subpart 13.5— Simplified Procedures for Certain Commercial Products and Commercial Services; and FAR Part 12—Acquisition of Commercial Products and Commercial Services, and is expected to exceed the simplified acquisition threshold. (iii) The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular (FAC) Number 2023-04, with effective date June 2, 2023. (iv) The associated NAICS code 541690 and the small business size standard is $19 million. This requirement is full and open with no set-aside restrictions. (v) This requirement is for the following services: CLIN 01: BASE PERIOD (YEAR 1) - Research Support Services for the Impact of Rare Diseases on Patients and Healthcare Systems (IDeaS) initiative Pilot Program. CLIN 02: OPTION PERIOD 1 (YEAR 2) - Research Support Services for the Impact of Rare Diseases on Patients and Healthcare Systems (IDeaS) initiative Pilot Program. (vi) The Statement of Work (SOW) with a description of the services to be performed is included as Attachment 1 to this Solicitation. (vii) The Government anticipates award of a time-and-materials type contract for this acquisition, and the anticipated Base Period of Performance is September 25, 2023, through September 24, 2024. It also includes one (1) successive 12-month option period. (viii) The Solicitation provisions and clauses are included as Attachment 2. FAR clause 52.212-5 is included as Attachment 3. Invoicing instructions are included as Attachment 4. (ix) Instructions for Quoters and Evaluation Criteria and Procedures are included as Attachment 5. (x) Quoters are to include a completed copy of the provision at FAR clause 52.212-3, Offeror Representations and Certifications—Commercial Products and Commercial Services (Dec 2022), with its quotation. If the Quoter has completed FAR clause 52.212-3 at www.sam.gov, then the Quoter does not need to provide a completed copy with its quotation. (xi) There are no additional contract requirement(s) or terms and conditions applicable to this acquisition. (xii) The Defense Priorities and Allocations System (DPAS) are not applicable to this requirement. (xiii) Responses to this solicitation must include clear and convincing evidence of the Quoter’s capability of fulfilling the requirement as it relates to the technical evaluation criteria. The price quotation must include the labor categories, an estimate of the number of hours required for each labor category, fully loaded fixed hourly rate or each labor category, breakdown and rationale for other direct costs or materials, and the total amount. The Unique Entity ID from SAM.gov, the Taxpayer Identification Number (TIN), and the certification of business size must be included in the response. All quoters must have an active registration in the System for Award Management (SAM) www.sam.gov. Fax responses will not be accepted.
