This contract is for a firm-fixed-price purchase order for Certified Medical Assistant (CMA) and Licensed Practical Nurse (LPN) services at the Warm Springs Health and Wellness Center in Warm Springs, Oregon. The contractor will provide clinical nursing and medical support including patient assessments, vital signs, patient education, care coordination, and documentation in the electronic health record. The period of performance is one year from the date of award, with clinic hours generally 8:00 AM–5:00 PM Monday through Friday (except Wednesday starting at noon).
