This opportunity is for a one-year Firm-Fixed-Price purchase order to provide supplemental dental hygienist and dentist services at the Warm Springs Health and Wellness Center in Warm Springs, Oregon. The contract aims to ensure continuity of oral health services and improve patient access to care for the Warm Springs Service Unit. Services will be performed on-site at 1270 Kot-Num Road, Warm Springs, OR 97761, with a clinic schedule generally running Monday through Friday. The contractor must hold relevant, unrestricted licenses and certifications, including current Basic Life Support (BLS), and will be evaluated on experience, price, and the ability to provide timely, reliable services.
