Solicitation no. 75H71323Q00039 has been issued as a Request For Quotation for a firm fixed price commercial items supply contract for custodial supplies located at Chemawa Indian Health Center, Western Oregon Service Unit, Salem, OR at one lump sum price for items on the price schedule (see Attachment 1-Schedule of Items). The performance period includes a one-year Base plus four (4) 1 -year option years. The required delivery date is December 31, 2027 for including option terms. The delivery location and place of acceptance is Chemawa Indian Health Center, Western Oregon Service Unit 3750 Chemawa Rd NE Salem, OR 97305, F.O.B. Destination. The preliminary notice does not commit the Government to an extension. (b) If the Government exercises this option, the extended contract shall be considered to include this option clause. (c) The total duration of this contract, including the exercise of any options under this clause, shall not exceed 5 years. As used in this clause - Payment request means a bill, voucher, invoice, or request for contract financing payment with associated supporting documentation. The respondent shall enter the unit price and extended totals for each item listed on attachment 1. If applicable, enter “equal item” manufacturer and manufacturer item number. The price shall include necessary supervision, management, labor, transportation, equipment, materials, shipping and any other direct incidentals costs, overheard and profit, unless the cost is identified as separate priced contract line item Award will be made to the responsible respondent pursuant to FAR subpart 9.1.
