Currently, the Yakama Indian Health Center has a Purchase Agreement with a vendor to provide linen and laundry services. The Yakama Indian Health Center is an ambulatory care center that provides services primarily to the members ofthe Yakarna Nation (Tribe) and other members ofnationally recognized tribes. 2.0 Location: The YIHC is located at 401 Buster Road, Toppenish, Washington. 3.0 Contact: For questions regarding the solicitation and contracting ofservices, please contact: Stacy Hill, Purchasing Agent 509-865-2102 For technical questions and point ofcontact after contract award, please contact: John Pulsipher, P.E., Facility Manager 509-865-1716 Or Louis Gunnier, Maintenance Supervisor 509-865-1777 4.0 Services to be Provided Weekly Services to be Performed by Vendor: Provide and launder towels of various sizes Provide and exchange weekly antifatigue mats for the Lab area Provide and exchange monthly carpeted mats of various sizes throughout the facility Provide and launder microfiber towels, dust mop, and wet mops Provide and launder thermo blankets Provide and launder patient gowns Provide and launder protective lab coats for Dental and Lab departments Provide and launder shirts and pants for Maintenance personnel Provide and launder scrubs for Housekeeping personnel Maintain air freshener system in all bathrooms Launder curtains owned by IHS as needed Government Furnished Property: Property: Laundry from each medical facility will be clearly labeled so the Contractor can return laundered items to the proper location. Informational Notice: In accordance with Contract Clause FAR 52.xxx-x the Contractor shall reimburse the Government for any damage to Government property. Contractor Furnished Property: Property: Except for the property specified in paragraph 5.0 as Government furnished, the Contractor shall provide all facilities, equipment, transportation, scales and supplies (example: laundry detergent, packaging and labeling materials, dry .cleaning solutions, etc.). Facility Inspections: The Government reserves the right to conduct inspections ofthe contractor's laundry facilities at any time as authorized in accordance with FAR 52.246-4. 7.0 Schedule ofWork: The work may be performed from 7:00 a.m. to 5:00 p.m. during normal government business days, excluding federally recognized holidays.
