This solicitation has been prepared with the intent to award a Firm Fixed Price (FFP) contract, utilizing procedures under FAR part 13. The proposed contract action is for one (1) base year plus four (4) option years to provide non-personal service for the proper collection, transportation and disposal of biohazardous medical waste services at Phoenix Indian Medical Center, 4212 North 16th Street, Phoenix, AZ 85016 and Sherman Indian Health Center, 9010 Magnolia Avenue, Riverside CA, 92503. This announcement constitutes the only solicitation. Quotations are being requested and a written solicitation will not be issued. The Request for Quotation (RFQ) number is 75H71222Q00007 issued as a Request for Quotation (RFQ). This solicitation documents and incorporates provisions and clauses in effect through FAR FAC 2023-01 December 30, 2022. It is the contractor's responsibility to be familiar with the applicable clauses and provisions. The clauses may be accessed in full text at this address: https://www.acquisition.gov/. The North American Industry Classification System (NAICS) Code for this acquisition is 562112, with a size standard of $41.5M. The Government will award a firm, fixed price contract resulting from this combined synopsis/solicitation to the responsive and responsible offeror whose conforming response is determined to provide the best value to the Government, price and other factors considered. The Government has determined best value is achieved by selecting the proposal that is technically acceptable and pricing is fair and reasonable for the Government to pay. The Government will only consider firm fixed-price quotations from SBA Small Business Concerns. Offeror to be considered responsible shall have an updated data and information in System for Award Management (SAM), FAR provision 52.212-3 Offeror Representations and Certifications-Commercial Items (Dec 2022). Indian Health Service (IHS), Phoenix Area Office on behalf of Colorado River Service Unit is conducting this Sources Sought to identify contractors who possess the capabilities to provide non-personal linen rental services at Parker Indian Health Center, 12033 Agency Road, Parker, AZ 85344. The designated NAICS code is NAICS: 812331 Linen Supply and PSC Code is S209 - Housekeeping- Laundry/Dry Cleaning. Scope of Work The contractor shall provide qualified personnel, supplies, materials and equipment necessary to provide LINEN RENTAL SERVICE in accordance with the linen requirement as may be needed to meet the needs of the Indian Health Service, Parker Indian Health Center, 12033 Agency Road, Parker, Arizona 85344. Linens Rental Service – All items shall be laundered in accordance with the Standards as established by the American Institute of Laundering and the Joint Commission on Accreditation of Health Care Organization. Contractor will provide on a weekly basis clean and dry: 75 each microfiber wet mops Large 1000 each 14” x 16” micro tech towels 100 each 18” microfiber pads Inspection of delivery items shall be made by the Housekeeping Linen Department upon each delivery. Periodically during the term of the contract, the Contractor’s plant shall be available for inspection by representative off the US Indian Health Service. The Contractor’s equipment and facilities must comply in accordance with the JCAHO Standards for the satisfactory performance of the services to be required under this contract. Such equipment and facility shall be subject to inspection by the Contracting Officer or his/her designee. The articles furnished by the Contractor shall be inspected to determine that they meet the requirements of the contract and other necessary standards. The quality of articles furnished hereunder must be maintained. Articles showing permanent stains of sign of wear, fraying, tears, etc., must be withdrawn and new material supplies at additional cost to the Government. If the Government is at fault in mishandling linen articles furnished by Contract, due theft, loss or damaged line, the government will reimburse up to 50% for replacement costs on linen articles reverenced in inventory listing. PERIOD OF PERFORMANCE: April 1, 2022 - March 31, 2027 DELIVERY AND PICKUP: Delivery of clean linen and pick-up of soiled linen shall be accomplished once a week on Mondays. Hours of delivery and pick-up shall be between 6:00 AM and 11:00 AM. If a major /Federal holiday falls on Monday delivery pick-up shall be made on the next business day.
