The Period of Performance is a base period plus one (1) option period (six (6) months per period): Base Period: 11/01/2025 through 04/30/2026 1st Option Period: 05/01/2026 through 10/31/2026 The North American Industry Classification System or NAICS code is 621111 – Offices of Physicians with a small business size standard of $16.0 Million. Evaluation factors may include: Qualifications of the Vendor, technical capability, and approach in order to meet the government’s requirement as defined in the Performance Work Statement (PWS). Experience (Internal Medicine Key Personnel) Key Personnel’s Licenses/Certifications – BLS, ACLS, PALS, Current Medical License, and DEA License.
