The consultant shall provide technical assistance to assess and improve the Case Management Program across three healthcare facilities: Northern Navajo Medical Center, Dzilth-Na-O-Dith-Hle Health Center, and Four Corners Regional Health Center. The Period of Performance for this requirement is November 1, 2025 to April 30, 2026. The acquisition is a 100% small business set aside.
