AMENDMENT 1: The subject solicitation is hereby amendment as follows: 1. The Schedule of Items now includes three different Procurement Agent levels. 2. Place of performance has been clarified. 3. Award will be based on lowest-priced, technically-acceptable quotes. 4. Qualifications for each level of position are included. 5. The local clause at NAIHS-ACQ-12 Personal Services Contract has been revised. 6. The PWS has been revised - changes highlighted in yellow. 7. The combined synopsis/solicitation has been revised - changes highlighted in yellow. All other terms remain the same. _________________________________________________________________________________________________________ This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in subpart 12.6, as supplemented with additional information included I this notice. This announcement constitutes the only solicitation; QUOTES are being requested and a written solicitation will not be issued. Solicitation no. 75H71025Q00152 is issued as a request for quotations (RFQ). The solicitation document incorporates provisions and clauses that are those in effect through Federal Acquisition Circular (FAC) 2025-03, effective January 17, 2025. The solicitation is a total Indian-owned, small business, economic enterprise (ISBEE) set aside under North American Industry Classification System (NAICS) code 541611 with an associated small business size standard of $24.5M. This is a personal service contract under the authority at 25 U.S.C. 1638c and is limited to individuals and organizations residing in the geographic vicinity of the health facility.
