The Indian Health Service, Gallup Indian Medical Center, located in/at 516 E Nizhoni Blvd, Gallup, New Mexico, 87301 has an upcoming requirement for six (6) non-personal service contracts: three (3) General Surgeons and three (3) Orthopedist services. The Period of Performance is a base period plus three (3) option periods (six (6) months per period): Base Period: 07/01/2025 through 12/01/2025 1st Option Period: 01/01/2026 through 06/30/2026 2nd Option Period: 07/01/2026 through 12/01/2026 3rd Option Period: 01/01/2027 through 06/30/2027 The North American Industry Classification System or NAICS code is 621111 Offices of Physicians (except Mental Health Specialists) with a small business size standard of $16.0 million. The Request for Quotations No. 75H71025Q00140 using the principles of FAR 12 Acquisition of Commercial Items in conjunction with FAR 13.5 Simplified Acquisition Procedures will be issued on or by May 1, 2025. Evaluation factors may include: 1. Technical capability and approach in order to meet the government’s requirement as defined in Attachment A & B – Performance Work Statement (PWS). 2. Key personnel shall meet the requirements listed in section 6.0 of the PWS. 3. Past Performance on three (3) projects providing non-personal service contract for General Surgeon and Orthopedists services within the past five (5) years. The Government reserves the right to retrieve and review past performance information from the Contractor performance Assessment Reporting System (CPARS) and other federal sources and years. 5. Price. Quotes will be due on or by May 15, 2025. The acquisition is a 100% Indian Small Business Economic Enterprise (ISBEE) set aside.
