The Indian Health Service (IHS), Crownpoint Healthcare Facility located in/at Crownpoint, NM 87313 is seeking capable sources for one (1) Clinical Laboratory Scientist (CLS) to perform a wide range of clinical laboratory test/examinations as requested by medical staff for use in clinical diagnosis, patient screening , monitoring patient therapy and status, and other authorized purposes. Services shall be performed in accordance wiht the attached, Performance Work Statement (PWS). The period of performance is for one year from date of award, or until the position is filled with a permanent hire. ***See attached SUBMISSION INSTRUCTIONS*** The Contractor shall provide an all-inclusive rate, and is resposiible for all cost associated with providing said servies, i.e. travel, per diem, lodging/housing , etc.. **The Contractor shall be responsible for houisng their respective provider, if no Government Housing is available.** Upon selection, the provider will be placed on a waiting listto acquire Governemtn quarters. If Government becomes available, the provider shall submit a housing application and make rental/payment arrangements with the CHF Housing Manager..We also recmend the Cities of Gallup, NM (55 miles) or Grants, NM (55 miles)..
