The Period of Performance will be as followed: Base Period: 10/30/2023 – 02/29/2024 1st Option Period: 03/01/2024 – 06/30/2024 2nd Option Period: 07/01/2024 – 10/29/2024 The North American Industry Classification System or NAICS code is 561320 Temporary Help Services with a small business size standard of $11M. The Request for Quotations No. 75H71024Q00001 using the principles of FAR 12 Acquisition of Commercial Items in conjunction with FAR 13 Simplified Acquisition Procedures will be issued on or by September 20, 2023. Evaluation factors may include: Board certified Obstetrician provider Contractor’s minimum experience of twenty-four (24) months. Complete candidate profile (profile must be submitted): Curriculum Vitae (CV), current board certified, Basic Life Support (BLS), Advance Cardiac Life Support (ACLS), possess a valid state driver’s license are ALL mandatory for all specialties for each proposed candidate. All medical health requirements/conditions of employment in section 6.4. The Contractor shall demonstrate: Competence in Obstetrics and Gynecology Confidentially of patient care and records Successful application for credentialing by the medical staff of GIMC Interview with Contractor against Statement of Duties, Performance Work Statement, and Contractor Profile. Review of references (at least 3) Certificate of Insurance (COI) Current Copy of Certificate of Insurance for each candidate Price Quotes will be due on or by October 2, 2023 by 8:00 a.m. MDT. The acquisition is unrestricted.
