The period of performance will be as follows: Base Period: 07/01/2023 – 09/01/2023 1st Option Period: 10/01/2023 – 12/31/2023 2nd Option Period: 01/01/2024 – 03/31/2024 3rd Option Period: 04/01/2024 – 06/30/2024 The North American Industry Classification System or NAICS code is 621111 with a small business size standard of $12.0 M. The Request for Quotations No. 75H71023Q00030 using the principles of FAR 12 Acquisition of Commercial Items in conjunction with FAR 13 Simplified Acquisition Procedures will be issued on or by 03/28/2023. Evaluation factors may include: Required capabilities and performance characteristics: 1. Residency in Anesthesiologist 2. Contractor’s experience of twelve (12) months. 3. Complete candidate profile (profile must be submitted): Curriculum Vitae (CV), current board certified, Basic Life Support (BLS), Pediatric Life Support (PALS) and Advance Cardiac Life Support (ACLS), possess a valid state driver's license are ALL mandatory for all specialties for each proposed candidate. All medical health requirements/conditions of employment in section 6.4. 4. The Contractor shall demonstrate: a) Competence in Anesthesiologist/CRNA. b) Confidentiality of patient care and records c) Successful application for credentialing by the medical staff of GIMC 5. Interview with Contractor against Statement of Duties, Performance Work Statement, and Contractor Profile 6. Review of references (at least 2) 7. Submittal of Option CLIN 8 Certificate of Insurance (COI) 9. Current Copy of Certificate of Insurance for each candidate 10. Price Quotes will be due on or by 04/10/2023.
