The period of performance will be as follows: 10/27/2022 - 01/26/2023 The North American Industry Classification System or NAICS code is 621111 with a small business size standard of $12.0 M. The Request for Quotations No. 75H71022Q00092 using the principles of FAR 12 Acquisition of Commercial Items in conjunction with FAR 13 Simplified Acquisition Procedures will be issued on or by 09/09/2022. Evaluation factors may include: Required capabilities and performance characteristics: 1. Residency in Obstetrics and Gynecology 2. Contractor’s experience of twenty-four (24) months. 3. Complete candidate profile: Curriculum Vitae (CV), current certification by the Basic Life Support (BLS), and ACLS mandatory for all specialties for each proposed candidate. 4. The Contractor shall demonstrate: a) Competence in inpatient and outpatient surgical care. b) Interview and examine medical patients, review past medical history, and request and/or perform diagnosis tests and examinations deemed necessary to obtain all possible information related to each case. c) Successful application for credentialing by the medical staff of GIMC d) Maintenance of professional certification through participation in appropriate programs for continuing medical and pharmaceutical. e) Competence in surgery procedures. 5. Interview with Contractor against Statement of Duties, Performance Work Statement, and Contractor Profile 6. Review of references (at least 2) 8. Current Copy of Certificate of Insurance for each candidate 9. Price 10. Submission of locums that could ONLY provide full-time coverage (NOT part-time) for 16 weeks and could start as soon as possible. Quotes will be due on or by 09/16/2022.
