SYNOPSIS The Indian Health Service, Gallup Service Unit, Gallup Indian Medical Center, located at 516 E Nizhoni Blvd, Gallup, NM, 87301, has an upcoming requirement for a Bulk Funded Blanket Purchase Agreement (BPA) for the purchase of Humipak Self Seal Pouches for use by the Dental Clinic of the Gallup Indian Medical Center on an “As Needed Basis”. The period of performance will be August 1, 2022 to July 31, 2025, in accordance with the delivery order schedule seen below: Base Year: August 1, 2022 to July 31, 2023 Option Year One: August 1, 2023 to July 31, 2024 Option Year Two: August 1, 2024 to July 31, 2025 North American Industry Classification System (NAICS) code is 339112 – Surgical and Medical Instrument Manufacturing with a small business size standard of 1,000 employees. The Request for Quotations No. 75H71022Q00052 using the principles FAR 13 – Simplified Acquisition Procedures (SAP) will be issued on or by July 11, 2022. Evaluation factors may include technical capability to meet the government need, delivery time, past performance, and price. Quotes will be due on or by July 18, 2022. The acquisition is a total Indian Small Business Economic Enterprise set aside.
