The period of performance will be as follows: 05/26/2022 – 09/10/2022 The North American Industry Classification System or NAICS code is 621111 with a small business size standard of $12.0 M. The Request for Quotations No. 75H71022Q00045 using the principles of FAR 12 Acquisition of Commercial Items in conjunction with FAR 13 Simplified Acquisition Procedures will be issued on or by 05/05/2021. Evaluation factors may include: 1. Contractor shall demonstrate: a) Competence in inpatient and outpatient obstetrical/midwifery care. b) Successful application for credentialing by the medical staff of GIMC c) Maintenance of professional certification through participation in appropriate programs for continuing medical and pharmaceutical education. 2. Contractor’s experience of twenty-four (24) months. 3. Contractor’s Licenses/Certifications (ACLS, medical license, Diplomas, ACLS, NRP, BLS etc.) 4. Current Board Certified eligibility or certification by the American Board of Midwifery. 5. Available full-time for 16 weeks 6. Able to start as soon as possible 7. Valid Driver's License 8. Two (2) References of the Contractor 9. Interview with Contractor against Statement of Duties, Performance Work Statement, and Contractor Profile 10. Current Copy of Certificate of Insurance 11.
