The Navajo Area Indian Health Service, Gallup Indian Medical Center located in Gallup, New Mexico is requesting for quotes to provide Exterminating and Pest Control Services to the Gallup Indian Medical Center compound in accordance with the attached Statement of Work (SOW) and the attached Service Contract Act WD #2015-5451, Pest Controller. PERIOD OF PERFORMANCE: The period of performance will be awarded as a base plus option year(s) listed below: Base Year: March 3, 2022 to March 2, 2023 Option Year One: March 3, 2023 to March 2, 2024 Option Year Two: March 3, 2024 to March 2, 2025 Option Year Three: March 3, 2025 to March 2, 2026 EVALUATION CRITERIA: The Government reserves the right to issue a single award or multiple awards to contractor(s) whose quote represents the best value as defined by FAR 2.101. In determining best value, Price and Evaluation Factors will be considered. The Evaluation Factors consist of: Experience, Past Performance, Qualifications, and Logistics. The socio-economic status is 100% set aside for Small Business Concerns Quotation Packets shall be submitted via electronically by email directly to the Contracting Officer with the Subject Line marked as 75H71022Q00027 to: stefanie.atene@ihs.gov. Limit offer size to 6MB. To be considered for the award, your offer shall include the following documents or it will be non-responsive and removed from award consideration: Price. Fee Schedule found at #5 of the SOW will identify the total price per year. Provide a Quotation summary and an outline of prices per subject item on a separate sheet that details how the pricing was determined. Example: an itemized quotation of single prices that add up to the total amount. Include supplies, materials, etc. Experience: Provide a list of at least (3) past projects of similar scope; include detailed project description, location, dollar amount, and period of performance. Past Performance: Provide a letter of reference from the most recent customer. Qualifications: Provide evidence that you are qualified to do the work. Certificates, Licenses, SBA Certifications, etc. Logistics: Provide a plan of action describing the logistics of how the services will be provided.
